We're improving people's health.
Many serious health conditions can be prevented when we support and encourage individuals at risk to change their behavior. United Way is providing access to solutions for obestity, diabetes, hypertension, tobacco use, and substance abuse.
How do we know it's working?
Community programs are evaluated considering whether they are reducing avoidable ER visits, hospitalizations or institutionalizations. Condition-specific health indicators are measured for improvement.
What does it mean for our community?
A healthy community avoids much of the high, personal and monetary costs of preventable illness. Healthy adults are less likely to need emergency treatment for preventable diseases and illnesses, and are better able to care for themselves as they age.
Our community's health agenda is evidence that working together, we can do more than anyone of us can do, on our own.
